South Tyneside Health Collaboration is excited to announce a new provision that will target those at risk patients that are reluctant to attend general practice.
The Health and Wellbeing Service aims to deliver a community outreach programme to ensure effective, proactive case finding for people at risk of developing long term conditions or who are currently living with an undiagnosed condition.
The service is aimed at improving accessibility for vulnerable patients by offering health checks, educational sessions for health and wellbeing, pathways into GP practices and more. It is aimed at those patients that may refrain from attending GP appointments
It will focus on population groups experiencing poorer than average heath access and those who are at a high risk of poor health and this will run in line with the Core20 plus 5.
Our Health and Wellbeing Team will be targeting patients in South Tyneside via a robust marketing campaign with a leaflet drop locally and posts on social media. The Team will also provide practices with posters, leaflets and information on the drop-in session available.
