Service Delivery

Our services are constantly growing as we continue to work in partnership with the local heal and care system. Some of the services currently provided by STHC include:

Covid Vaccination Programme

South Tyneside Health Collaboration has proudly delivered the Covid-19 vaccine programme for the residents of South Tyneside since December 2020. The programme has administered over 240,000 covid vaccines to care home residents, housebound patients, NHS staff and the general public during the uncertain times of the pandemic and now.
We have employed and trained numerous healthcare and admin staff to meet the demand and urgency of the programme, also established and staffed our dedicated Covid-19 vaccine call centre where patients can call for vaccination bookings and advice. We have clinics located at Flagg Court Medical Centre and the Glen Medical, we have also delivered vital outreach events where we have administered vaccines in churches, charities and public venues with high footfall to vaccinate those that are hard to reach.

Care Home Team

The EHCH team deliver DES service requirements to patients residing in Care Homes on behalf of the practices in South Tyneside.  They follow the EHCH framework and are in direct contact with Commissioners, the Local Authority and the ICB.  Each member of the team has an identified area to work on for the Enhanced Health in Care Homes framework.

The team offer proactive care to patients living in Care Homes, organising, and updating DNACPRs and EHCPs and carrying out annual reviews on behalf of practices. The team also organise and attend monthly MDTs alongside GPs and other health care professionals who have involved in residents care. During MDT meetings that are held at the care homes, most residents will have detailed review including structured medication review, reviewing QOF indicators if still needed and residents face to face assessment an examination by GP. The Nurse Practitioners carry out weekly home rounds and often have unplanned consultations, dealing with acutely unwell residents therefore alleviating pressure on GPs.

Each member of the team including nurse practitioners and care coordinators is leading on a clinical area such palliative care, safeguarding, falls, oral health and vaccinations. 

EHCH team are supporting GP practices by providing planned and acute care to residents also supporting community and practice nurses by delivering vaccination. Also, they are providing training and education to the care home staff.

GP Advice Line

The Weekend Primary Care Support Service is currently made up of a small group of local practice GPs’ who work on rotation within the Community Nursing Team at Clarendon, Jarrow. The service currently offers instant access to an experienced GP to provide clinical support and advice to Community Teams (nursing/ paramedic) and all Care Homes for South Tyneside residents. This is a bypass to using the ‘standard’ system of NHS111 and provides access to required prescriptions and/or ambulance call outs or even A&E attendance.

Opening Hours are 9:00am-13:00pm, Saturdays and Sundays. Public Holiday opening times may differ.

Admiral Nurses Programme

The South Tyneside Admiral Nurse service improves the quality and experience of care for families affected by dementia. The service is available for people registered with a GP in South Tyneside with dementia and their families. Their role is to primarily support carers in their role and prevent breakdown, and also includes support through pre-diagnosis, education and support through the disease progression, into end of life, and post-bereavement.

The Admiral Nurses offer support with complex decision-making (such as planning a transition into care), or where the person with dementia also has other chronic conditions. The service also provides specialist advice and guidance to GPs, practice staff and other health and social care professionals, to support best practice in dementia care.

Enhanced Access

You can now book a medical appointments over the weekend or evening at a general practice in South Tyneside.

This is called the Enhanced Access Service.

A small number of specific surgeries will be open on different days and times to create more options for patients.

Who is providing this service?

The South Tyneside Health Collaboration has been asked by the local clinical commissioning group (ICB), the NHS organisation who pay for health services in South Tyneside, to run the service.

How do I get an appointment?

You can get an appointment by calling your normal GP practice during normal working hours. You may be asked some questions to check that it is the best service for your needs. All patients will have a 15-minute booked appointment slot. If you are late, it may not be possible to see you in that session and you will need a new appointment.

Call your usual registered practice for an appointment. If you need to cancel it, can’t find us or require help, do call us directly and we will try to help, otherwise do call your practice or 111 if we are both closed. You will not be able to leave a message with us.
